Download and print Certificate of Authenticity Form.3. Record an audition tape in front of a witness.4. Sign the Certificate of Authenticity Form and have your witness sign also.5. Mail in above items with the proper application fee within two weeks. ($40 until regular audition date, $60 afterwards)Once you apply, you MUST follow through above procedure.
